Test Instructions

The Free PSA test measures the level of unbound (free) Prostate-Specific Antigen circulating in your blood. PSA is a protein produced by cells in the prostate gland. It exists in your bloodstream in two forms: attached to other proteins (bound) or floating freely (free). While a standard PSA test measures the combination of both forms, measuring free PSA levels acts as a crucial secondary screening tool. Calculating the ratio or percentage of free PSA compared to total PSA helps pathologists and urologists more accurately differentiate between benign prostate conditions and active prostate malignancies, significantly reducing the need for unnecessary, invasive tissue biopsies. 🩺🛡️

When is this test performed and for which diseases? 🤔

  • To Differentiate Borderline Screening Results: This test is performed when a patient’s Total PSA results fall into a borderline “gray zone” (typically between 4.0 ng/mL and 10.0 ng/mL) to evaluate the high probability or risk of Prostate Cancer. 🚨🔍

  • To Rule Out Non-Cancerous Enlargement: It is ordered when a man experiences urinary issues like weak flow or frequent nighttime urination to check if the elevated total PSA is simply due to Benign Prostatic Hyperplasia (BPH). 🛑

  • To Identify Prostate Inflammation: This examination is performed to rule out acute or chronic Prostatitis (bacterial or non-bacterial inflammation of the prostate gland), which can cause temporary spikes in total PSA while keeping the free PSA percentage relatively high. 🧼🏥

  • To Monitor Post-Treatment Success: It is performed as a follow-up assessment for patients undergoing active surveillance or tracking treatment efficacy for diagnosed Prostate Malignancies. 📊

Test Specifications: 📋

  • Sample Type: Blood (Serum) 🩸

  • Preparation: No fasting is required. However, you must avoid ejaculation, heavy bicycling, or undergoing a digital rectal exam (DRE) for at least 48 hours prior to the blood draw, as these activities can cause mechanical pressure on the prostate and falsely elevate your baseline PSA levels. 🚴‍♂️❌☀️

  • Reporting: High-precision quantitative hormone/tumor marker panel results are typically delivered within 24 hours. ⏱️
